Discovered in the 1960s, verapamil is among the most widely prescribed calcium channel blockers worldwide for management of angina, hypertension, and certain tachyarrhythmias. It is a phenylalkylamine calcium channel blocker with high selectivity for L-type channels and notable effects on cardiac conduction and vascular tone.
Verapamil inhibits calcium entry into vascular smooth muscle and cardiac myocytes, producing coronary and peripheral vasodilation and reduced myocardial contractility. It is available in oral and parenteral forms and is typically initiated at low doses with gradual titration according to response, tolerability, and comorbidity profile.
Primary indications include chronic stable angina and vasospastic (Prinzmetal) angina, as well as essential hypertension where noninvasive blood pressure reduction is desired. It provides rate control in certain supraventricular tachyarrhythmias and is used to slow ventricular response in atrial fibrillation or flutter when beta-blockers are inappropriate or not tolerated.
Secondary indications encompass selected off-label uses and guideline-supported alternatives. Verapamil can be used for short-term arrhythmia suppression in emergency settings where AV nodal blockade is desired. In experienced centers, it is employed for cluster headache prophylaxis and, in some patients, migraine prophylaxis. Caution is advised in patients with heart failure with reduced ejection fraction or significant conduction disease, where negative inotropy may worsen symptoms.
Verapamil blocks L-type calcium channels in cardiac and vascular smooth muscle. Reduced calcium entry leads to decreased contractility of the heart and relaxation of vascular smooth muscle, yielding lower heart rate, decreased blood pressure, and dilated coronary arteries. In the AV node, slowed conduction prolongs refractoriness, which helps control rapid ventricular responses in atrial arrhythmias.
Common adverse effects include constipation, dizziness, flushing, edema, and fatigue. Hypotension and bradycardia may occur, as can conduction abnormalities such as AV block. Verapamil can worsen heart failure with reduced ejection fraction due to negative inotropy and should be avoided in severe systolic dysfunction. Caution is required when coadministered with beta-blockers or digoxin; monitor for interactions and signs of hypotension or bradycardia. Seek prompt medical attention for chest pain, syncope, or signs of heart block.
