What Semaglutide is

Hyperglycemia in adults with type 2 diabetes and obesity-related weight gain are conditions commonly targeted by incretin-based therapy. Semaglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist used by clinicians as a once-weekly subcutaneous injection and, in an oral formulation, for select indications. It is a synthetic analogue of human GLP-1 with modified pharmacokinetics to prolong activity.

Semaglutide binds GLP-1 receptors, enhances glucose-dependent insulin secretion, suppresses inappropriate glucagon release, and slows gastric emptying. These actions promote improved glycemic control and, at higher doses, reduced body weight through appetite suppression.

What it is used for

In adults with type 2 diabetes, semaglutide is used as an adjunct to diet and exercise to improve glycemic control when metformin alone is insufficient or as part of combination therapy.

In obesity management, higher weekly doses of semaglutide are approved for chronic weight reduction in adults with a body mass index of 30 kg/m2 or greater, or 27 kg/m2 with weight-related comorbidity, alongside lifestyle measures.

Contraindications and precautions

Contraindications include hypersensitivity to semaglutide or any component; personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN2); and pregnancy or lactation. Use in pediatric patients is not established.

Precautions include assessment for pancreatitis and gallbladder disease, with discontinuation if pancreatitis is suspected. Caution is advised in patients with severe gastrointestinal disease, delayed gastric emptying, or a history of significant dehydration. Retinopathy progression has been observed with some glucose-lowering therapies and warrants monitoring during treatment initiation and dose adjustment. Renal function should be monitored if dehydration occurs from GI side effects. Inform patients about potential thyroid C-cell effects that are reported in animal studies but have not established significance in humans. Initiate treatment with gradual dose escalation and monitor for adverse GI symptoms; suspend if severe events occur.

Side effects by frequency and interactions with other substances

Common adverse effects include gastrointestinal symptoms such as nausea, vomiting, and diarrhea, especially after initiation or dose escalation, along with decreased appetite, constipation, and injection-site reactions. Headache and fatigue are also reported. These effects typically diminish with continued therapy.

Occasional events include dizziness, abdominal pain, and dyspepsia. Rare but serious adverse events include acute pancreatitis, cholelithiasis or cholecystitis, and hypersensitivity reactions. Hypoglycemia may occur when semaglutide is used with insulin or sulfonylureas; risk increases with higher doses or renal impairment.

Drug interactions: semaglutide may delay gastric emptying and thereby affect absorption of concomitant oral medicines. Separate administration of certain medications as clinically indicated can reduce interaction. Caution with medications requiring rapid oral absorption; monitor for reduced efficacy of oral contraceptives or antibiotics in rare cases; adjust timing as needed. No major metabolic interactions with most concomitant agents have been identified.
