Dapsone is a sulfone antibiotic belonging to the diamino sulfone class. It inhibits dihydropteroate synthase in bacterial folate synthesis and exerts anti-inflammatory effects; its primary therapeutic uses include dermatitis herpetiformis and leprosy.
Dapsone is a synthetic agent used in certain bacterial infections and inflammatory skin conditions. In bacteria, it blocks folate synthesis; in the skin, it dampens neutrophil-driven inflammation. It is commonly given as oral tablets and dosed by indication and tolerance.
It treats dermatitis herpetiformis and leprosy as part of multidrug regimens. Clinically, it also serves as Pneumocystis jirovecii pneumonia prophylaxis for patients who cannot tolerate first-line agents.
Contraindications include known hypersensitivity to dapsone or other sulfone drugs. It is avoided in severe anemia and in known G6PD deficiency due to risk of hemolysis and methemoglobinemia. Use in pregnancy and breastfeeding requires careful risk–benefit discussion; dapsone crosses the placenta and may affect the fetus. In newborns, special caution is advised if exposure occurs.
Precautions include baseline blood counts and regular monitoring for anemia or neutropenia, as well as liver and kidney function tests. Watch for signs of methemoglobinemia, such as headache or shortness of breath with cyanosis. Patients with liver disease, older adults, or poor nutritional status may be more vulnerable to toxicity. Seek urgent medical care if you develop a rapidly worsening rash, fever, or breathing difficulties.
Common: anemia, methemoglobinemia, rash or itching, headache, nausea, vomiting, abdominal discomfort. Less common: dizziness, pale skin, mild fever, or transient liver-test elevations. Rare: severe skin reactions, drug reaction with eosinophilia and systemic symptoms (DRESS), agranulocytosis, severe liver injury, or neuropathy.
Rifampin can enhance dapsone metabolism and lower its levels; concurrent use requires monitoring and possible dose adjustments. Probenecid may increase dapsone exposure. Drugs that cause methemoglobinemia or oxidative stress—such as certain nitrates or local anesthetics—may raise the risk when used with dapsone. Avoid or monitor carefully with other bone-marrow or liver-suppressing drugs. Always inform your clinician about all medicines, supplements, and herbal products you take.
