Discovered in the early 1960s, amitriptyline is one of the oldest and most widely prescribed tricyclic antidepressants. It works by increasing levels of norepinephrine and serotonin in the brain and by dampening pain signals.
Amitriptyline is taken by mouth in tablet or liquid form. It is started at a low dose and gradually increased under medical supervision. It is not suitable for everyone, including people with certain heart conditions or glaucoma.
Primary use is for major depressive disorder, where mood, sleep, and energy improve with treatment. It can help people who have persistent fatigue, poor concentration, and low mood that do not respond quickly to other therapies.
Secondary indications include chronic neuropathic or musculoskeletal pain, such as diabetic neuropathy or fibromyalgia, where it can reduce pain and improve sleep. It is also used to prevent migraines and to manage functional GI symptoms, anxiety, and sleep disturbance in mood disorders. Some uses are off-label, guided by clinician judgment and patient needs.
Amitriptyline mainly blocks the reuptake of norepinephrine and serotonin, helping brain signaling and mood. It also interacts with histamine and acetylcholine receptors, which influence sleep and sensation.
Anticipate that antidepressant benefit often takes weeks. Receptor interactions contribute to both therapy and side effects, such as drowsiness or dry mouth, and help explain why sleep may improve before mood.
Use under medical supervision. Do not combine amitriptyline with monoamine oxidase inhibitors without guidance. Caution is advised for heart disease, glaucoma, seizures, or a history of suicide attempts.
Common side effects include drowsiness, dizziness, dry mouth, constipation, blurred vision, and weight change. Less frequent effects can involve urinary difficulties, low blood pressure on standing, or confusion in older adults. Seek medical advice if you notice abnormal heartbeats, severe dehydration, or signs of serotonin syndrome with other serotonergic drugs.
