Losartan is an angiotensin II receptor blocker (ARB). It blocks the angiotensin II type 1 receptor to reduce blood vessel constriction and lower blood pressure, with primary use in hypertension and kidney protection in diabetes.
Taken by mouth as tablets, it is usually prescribed for once-daily dosing and can be used alone or with other antihypertensive medicines. If you miss a dose, take it as soon as you remember unless it is almost time for your next dose.
Dosing is tailored to blood pressure and kidney function, and patients should not double a missed dose. Some people experience dizziness after the first dose or when the dose is raised.
Losartan lowers high blood pressure, helping prevent heart and kidney complications over time. Contraindications include pregnancy and a history of hypersensitivity to losartan or any tablet component. Using losartan during pregnancy can harm the fetus, so women who are pregnant or planning pregnancy should avoid it.
Precautions focus on kidney function and electrolytes. Losartan can raise potassium levels, especially in kidney disease, the elderly, or those taking potassium supplements or salt substitutes. Regular blood tests may be needed to monitor potassium and kidney function.
Other cautions include liver disease and dehydration. Tell your clinician about all medicines you take, since NSAIDs, diuretics, or other blood pressure medicines can alter effects and safety. Not recommended for children under 6; dosing in older children follows pediatric guidelines. If you may become pregnant, discuss options with your provider before starting therapy.
Common effects include dizziness or lightheadedness, particularly after starting therapy or after dose increases. Fatigue and mild nasal congestion or sore throat can also occur, and dizziness can contribute to falls in older adults.
Less common effects include stomach upset, back pain, and occasional changes in taste. A dry cough is less frequent than with ACE inhibitors but can occur in some people. Rarely, skin rash or itching may appear.
Serious but uncommon risks include angioedema, which presents as swelling of the face or throat and requires urgent care, and hyperkalemia with symptoms such as muscle weakness or palpitations. Very rarely, liver injury or kidney issues appear; report unusual symptoms promptly.
NSAIDs such as ibuprofen can blunt losartan’s blood-pressure lowering effect and may worsen kidney function, especially when hydration is poor. Using diuretics together can increase the risk of too-low blood pressure. Alcohol can heighten dizziness.
Potassium‑sparing medicines, potassium supplements, and salt substitutes containing potassium can raise potassium to dangerous levels. Reduced dosing or close monitoring is common when these are used with losartan, particularly in kidney disease or older adults.
Lithium and some antidepressants may have altered levels when combined with losartan. Aliskiren is not recommended with losartan in patients with diabetes or kidney impairment.
